Sonocystography Following Open Radical Retropubic Prostatectomy (ORRP)

ORLANDO, FL (UroToday.com) - Dr. Herbert Lepor and colleagues report their experience with 51 patients who underwent sonocystography on the eighth day following open radical retropubic prostatectomy (ORRP).

The technique involves volume measurements of a pelvic fluid collection before and after 150cc of saline are instilled into the bladder through the indwelling foley. The authors report that if the pelvic collection volume was < 132 cc and if the change in volume following instillation of saline was 37 cc or less, then there was no anastomotic extravasation, as documented by standard cystography.

The authors propose that anastomotic leaks result from disruption of the vesicourethral anastamosis by post-operative pelvic hematoma. The authors believe that extravasation of fluid into the pelvic hematoma is that which is being measured with the sonocytography technique. They submit that sonocystography should replace conventional gravity cystography in the post-ORRP setting, as this technique affords a "more quantitative assessment of extravasation."
